  • A Care Plan in Health Cloud is a plan made for each patient to guide them towards better health.

  • A Task is a specific action or step to help achieve a goal (like buying low-sodium food).

Example: Kidney Stone Problem

  • Imagine a patient with kidney stone issues.
  • The goal could be: Don’t get more kidney stones.
  • Some sub-goals might be:
    • Reduce salt intake.
    • Cut down on eating red meat.
    • Avoid a high-protein (keto) diet.

Setting Goals in Health Cloud

  • You can create specific goals like Reduce salt intake and set their priority by clicking on the New Goal button.

  • Multiple goals can be added under the main health problem.
  • Each goal helps track progress for the patient’s specific needs.
  • When you created Goal for this care plan, it's should be visible in the Goals list like the below figure

Creating Tasks For Each Goal

  • For each goal, you add tasks that help the patient achieve that goal by clicking on the New Task button.

  • For Reduce salt intake you might add tasks like:
    • Buy food labeled low sodium.
    • Avoid fast food (like burgers with high salt).
  • The system allows you to assign tasks to specific people, like a family member.
  • When you add the task, it's look like the below figure

Assigning Tasks and Responsibility

  • Tasks can be assigned to the patient themselves or caregivers (like their son or another family member).
  • This makes sure everyone knows what they need to do to help the patient stay healthy.

  • It’s a good idea to cook meals at home to control salt and ingredients.
  • Tasks should be small, clear actions (like “buy low sodium beef stock,” “eat red meat only twice a day”).
  • Make sure everyone in the care team knows their task for smooth care coordination.
